Telehealth Consent

1. What telehealth is
Telehealth means delivering healthcare using electronic communications, such as video visits and secure messaging, when you and your physician are not in the same location. Your NorevX care, including your initial evaluation, is delivered by telehealth by physicians licensed in California, consistent with California Business & Professions Code § 2290.5.
2. Expected benefits
Access to physician care without travel, faster scheduling, continuous protocol management, and secure follow-up communication with your care team.
3. Possible risks
As with any healthcare service, there are potential risks, including: technology failures may interrupt or delay care; in rare cases, insufficient information transmitted electronically may limit clinical decision-making; and security protocols could fail, causing a breach of privacy. Your physician may determine that telehealth is not appropriate for your situation and refer you to in-person care.
4. Your rights
You may withhold or withdraw this consent at any time without affecting your right to future care or treatment. You may ask questions about telehealth before consenting. All existing confidentiality protections under California and federal law apply to telehealth services. You have the right to access your medical records as provided by law.
5. Alternatives
You may choose to seek equivalent in-person care from a provider of your choice instead of, or in addition to, telehealth care through NorevX.
6. Consent
By checking the consent box in the NorevX assessment, you confirm that you have read and understood this consent, you are a California resident aged 18 or older, and you voluntarily consent to receive care through telehealth. A copy of this consent is retained in your medical record.
